$100 Incentive for COVID shot

Wisconsin DHS announces major program for limited time to help stop the spread

Dear iCare Members,

You may have heard that Governor Tony Evers and the Wisconsin Department of Health Services (DHS) announced Wisconsin residents who get their first dose of any COVID-19 vaccine from a vaccine provider located in Wisconsin beginning August 20 through September 6 can receive a $100 Visa® Gift Card. Wisconsin residents ages 12 and over are eligible for the reward and can request the reward by going to the Reward Sign-up page or by calling 844-684-1064 (language assistance is available). Proof of insurance, citizenship, ID is not required to be eligible for or to receive the $100 gift card.

In order to receive the reward, Wisconsinites ages 12 and older will need to have received their first COVID-19 vaccine within the period of August 20 through September 6 and submit the request for the $100 Visa Gift Card on the DHS Reward Sign-up page at https://100.wisconsin.gov/. Information submitted in the form will be used to verify you received your shot.

Let's answer a few questions that have been asked

Q: How long does it take to get your Visa card from Wisconsin DHS?

Q: Where can I can find vaccine locations near me?

A: To find a COVID-19 vaccine location in your community, visit Vaccines.gov or call 211 or 877-947-2211.

 

Q: I prefer a phone more than a computer. What can I do? 

A: If you do not have a valid email address or you need assistance filling out the form, call 844-684-1064, Monday through Sunday, 7 a.m. to 7 p.m. CST. Spanish, Hmong, Chinese Mandarin, Hindi, and Somali language assistance is available.

 

Q: If I get vaccinated at the Veterans Administration or by Tribal Health Services, can I still earn a $100 Visa Gift Card?

A: Yes!
