You are eligible if:

  • You live in Adams, Brown, Calumet, Columbia, Crawford, Dane, Dodge, Door, Florence, Fond du Lac, Grant, Green, Green Lake, Iowa, Jackson, Jefferson, Juneau, Kenosha, Kewaunee, La Crosse, Lafayette, Manitowoc, Marinette, Marquette, Milwaukee, Monroe, Oconto, Outagamie, Ozaukee, Racine, Richland, Rock, Sauk, Shawano, Sheboygan, Trempealeau, Vernon, Walworth, Washington, Waukesha, Waupaca or Winnebago county
  • You receive Medicaid and Supplemental Security Income (SSI) or SSI-related benefits

Medicare beneficiaries or those enrolled in a Medicare HMO are also eligible, as long as you receive Medicaid or SSI benefits.

How Do I Apply For State Medicaid?
