Scope of Service

The Scope of Service defines requirements and expectations for the provision of subcontracted, authorized and rendered services. The services shall be provided in compliance with service expectations in the Agreement and Wisconsin licensing and certification standards, as applicable. Provisions of the Scope of Service supersede any other agreements, including agreements between the Enrollee and Provider, such as intake agreements.

All iCare Long Term Care contracts are in the process of being updated to include both Family Care Partnership and Family Care (branded as Inclusa). The integrated contracts will include a rate schedule on the Long-Term Care Provider Services and Payment page within the contract and will no longer appear as part of the Scope of Service. Until the integrated contract is received, please refer to the scopes below for service definitions and existing scopes for compensation information.
