Am I eligible to join Aurora CompleteCare?

You could be eligible if you:

• Live in our service area of Kenosha, Milwaukee, Ozaukee, Racine, Washington, or Waukesha county
• Are eligible for Medicare and Medicaid benefits OR eligible for Medicare cost-sharing assistance under Medicaid
• Have Medicare Parts A and B

Call us at 1-855-818-1131 to see if you qualify for the Aurora CompleteCare (HMO SNP). Calling this number will connect you with a licensed Medicare Benefits Consultant who can help you find out if you qualify for the healthcare benefits and services provided by the Aurora CompleteCare.

Enroll through an in-home visit or over the phone by calling 1-855-818-1131.

You can enroll over the phone Monday - Friday, 8:00 a.m. to 5:00 p.m.

Or, if you prefer, you can enroll in Aurora CompleteCare online. 

After your eligibility has been determined, we will send you a membership packet explaining how to access your Aurora CompleteCare Benefits and Services.
